- Provide your preferred pharmacy information
- Review and sign necessary forms
Your first appointment is designed to give you the best start with our team.
- Length: 40–60 minutes
- What happens:
- Medication reconciliation
- Health history review
- Labs or screenings if indicated
- Creation of a personalized care plan
Please arrive 10 minutes early and bring:
- A valid photo ID
- Insurance card
- Any relevant medical records
