We ask new patients to arrive 10 minutes early to allow time for check-in and verification.
Referrals & Authorizations
If your insurance plan requires prior authorization or a referral, our team will assist you in coordinating with your insurance provider or specialist.
Copays & Self-Pay Options
Copays and balances are due at check-in.
We accept credit/debit cards, HSA/FSA cards, and other common payment methods.
For patients without insurance, transparent self-pay rates are available. Please call us to learn more.
No-Show & Cancellation Policy
We value your time and ask that you value ours.
Cancellations should be made at least 24 hours in advance.
No-shows or late cancellations may result in a fee.
Repeated missed appointments may affect your ability to schedule future visits.
